Please mail this Form together with your refundable ticket(s) to the following address: OC Ticketing Center, Room 1313, 13/F, Office Building, Great China International Exchange Square, Fuhuayilu Road, Futian District, Shenzhen, Guangdong Province. Post Code: 518034. OC will not accept those Application Forms mailed after September 30th, 2011 (postmark date).
深圳大组委有权自行决定是否接受您的退票申请。深圳大组委将只按照门票的票面价格退款，对于您其他任何与退票相关的权利请求、损害赔偿、损失及费用，包括但不限于邮寄费用、退款利息，深圳大组委将不承担任何责任。
OC has the right to decide whether to accept your application of refund. OC will only refund the face value of the ticket(s); interest thereon will not be paid. OC shall not be responsible for any other claims, damages, losses or expenses related to your application of refund, including without limitation, the mail fares.
